Membership Fees

Our fees are simple and transparent, as they should be. No hidden fees or complex invoicing. You'll know exactly what you're paying and exactly what you're getting.

2023 monthly enrollment fee

Age 10-19 $30 per month

age 20-44$65 per month

Age 45-64$85 per month

Age 65+$100 per month

Additional expense may also be incurred if supplies such as splints, cast shoes or crutches are needed. There may be pathology costs associated with sending skin biopsies to the laboratory for further testing. There may also be a small charge for supplies related to injection procedures. Blood draw fee of $7 with all in-house lab testing. Any added costs are always discussed with patients prior to ordering any tests or treatments.

there is a one time enrollment fee of $85 per person or $150 per family

for those who cancel, then re-join, a $450 re-enrollment fee will be charged

fee for service will also be available to family members not enrolled but need same day visits.
